Approaches to and outcomes of chronic total occlusion (CTO) percutaneous coronary intervention (PCI) the left anterior descending artery (LAD) have received limited study. We examined procedural characteristics patients who underwent LAD CTO between 2012 2022 in a large multicenter PCI registry. The was target vessel 3,059 11,753 PCIs (26%). Overall technical success rates were 89% 88%, respectively. Technical most frequently achieved using antegrade wire escalation (AWE) (77% successful procedures), followed by dissection re-entry (ADR) (12%) retrograde approach (11%). majority study lesions located within proximal (38%) mid (55%) segment 4.6% first or second diagonal. distribution crossing strategies used segments was, as follows: (AWE 92%, ADR 16%, 24%), 97%, 17%, 14%), distal 95%, 8%), first/second diagonal 94%, 11%, 17%). similar various locations CTOs (p=0.683). incidence major adverse cardiovascular events 2% (n=60) (p=0.106). treated with high acceptable complication rates, more often crossed AWE, but required frequent use approach, especially for lesions.
